Need Case Manager (QSP) to interact with developmentally disabled adults, participate in staffings, develop and monitor Individual Service Plan, provide/document individual and group services and keep accurate records.  

Requirements:  B.S. in human services field and 1+ yrs exp.  with DD population, computer skills and clear & valid driving record.
Ability to obtain and maintain qualifications as a QSP and a driver under the PACE/IDOT requirements. Strongly prefer current QSP (formerly QMRP).

Salary:  Negotiable based on experience – min. $13.64/hour
